This is an Armstrong 17" - 42" Swivel/Tilt TV Wall Mount, model number 64238. It is designed to fit most TVs up to 66 lbs (29 kg) and is also compatible with most PC monitors. 
The mount swivels 180° and tilts 15° forward. 
It extends up to 10-1/2 inches from the wall and can be closed to be only 2-5/8 inches away from the wall. 
The product has a universal design that fits most VESA patterns up to 200/200.
